EXERCISE PHYSIOLOGY AND CHRONIC DISEASE MANAGEMENT


THE CDM REFERRAL PATHWAY

  1. Fax or email medical objects from your GP to Full Circle Wellness

  2. FCW will contact you to arrange a suitable appointment time.

  3. Initial consult with your FCW allied health provider.

  4. Communication with relevant stakeholders about intended care plan to ensure holistic and collaborative approach. This could include GP, specialists, organisations, carers, other allied health service providers.

  5. Implementation of therapy.

  6. At referral determined rate. Medicare bulk billing may apply.

In Australia, chronic disease is the leading cause of morbidity, with approximately 48% of Australians having at least one chronic health condition. Furthermore, chronic disease attributes to more than half of all potentially preventable hospitalisations. 

Evidence unequivocally demonstrates that appropriate exercise interventions aid in the management and impede the progression of many chronic health conditions and associated comorbidities. Importantly, optimal benefits are achieved when exercise is:

  • supervised 

  • individualised

  • incorporates follow up

  • utilises a behavioural approach; and

  • can be modified to accommodate patient comorbidities

DOES MEDICARE COVER THE COST OF EXERCISE PHYSIOLOGY?

Yes, if you have a chronic health condition you may be eligible for Medicare subsidised consultations with registered allied health professionals, including an exercise physiologist. These services are provided with a referral from your GP under the Chronic Disease Management Scheme (CDM), formerly known as the Enhanced Primary Care Scheme (EPC).

 All of our practitioners are registered to provide services under the CDM and we bulk bill our services under Medicare.  

Eligibility for CDM referrals is broad, but generally requires that you have a terminal condition or a condition that has been there or is likely to be there for more than 6 months. Areas of focus can include, but are not limited to:

  • Musculoskeletal rehabilitation for pre and post surgery or injury. 

  • Physiological conditions including blood pressure issues, elevated cholesterol, cardiac conditions, respiratory conditions, neurological conditions and digestive and renal conditions. 

  • Psychological conditions like depression and other mental health conditions.

  • Lifestyle conditions such as diabetes and obesity.

  • Weight loss and general exercise and diet prescription. 

  • Lifestyle modification and falls prevention.

  • Exercise intervention for goal specific tasks.
