HOW IT WORKS
Referral
Either through self-referral or via your provider, you can be enrolled into a fully funded Support at Home program.
Assessment
A healthcare professional will conduct your assessment to identify your needs and goals with you.
Sessions
Your 1:1 session will be held with one of our Accredited Exercise Physiologists, either at our clinic or in your home.
Tailored programs
A personalised program will be provided to you to help you maintain your strength, mobility, and independence.
